Why the milligrams depend on the vial

40 units is 0.4 mL. At the Zepbound concentrations that is 1.67 mg, 2 mg, 3.33 mg, 4 mg, 5 mg, 6.68 mg, 8 mg or 10 mg, depending on which vial the liquid came from. Two of those look like labelled strengths: 5 mg at 12.5 mg/mL and 10 mg at 25 mg/mL. Neither is a labelled draw. The 30 mg multi-dose vial at 12.5 mg/mL gives its 7.5 mg dose in 0.6 mL, and the 60 mg vial at 25 mg/mL gives 15 mg in 0.6 mL; taking 0.4 mL from either is a different dose from the one the label describes. This is the trap with converting marks back to milligrams: a number that happens to match a strength on the box is not evidence that the draw is correct.

The mistake to watch for. A friend's 40 units is not your 40 units. Concentrations differ between Zepbound vials and, for compounded products, between pharmacies. Compare doses in milligrams and convert each through its own label.
